The ground-breaking Care Coordination Service Centre (CCSC), which offers support for priority patients referred for planned specialist outpatient care has commenced operations in Cairns.

The CCSC is a joint effort between QAIHC, Cairns and Hinterland Hospital and Health Service and the Torres and Cape Hospital and Health Service.

A first for the state, the CCSC aims to improve the health and wellbeing of people who live in rural and remote areas across Far North Queensland, by providing health systems improvements strengthening care coordination.

The centre supports patients with complex health conditions and has been developed as a single contact point for patients to help them navigate the health system.

The centre has a range of co-located services such as patient travel, referral systems and digital information. It has also launched the Priority Patient Dashboard, a patient risk prioritisation process to provide greater visibility for priority patients.
